IMPORTANT: Following the publication of Order INT/96/2026 of 12 February (https://www.boe.es/eli/es/o/2026/02/12/int96), which extends the validity of the foreigner identity cards issued to persons affected by the conflict in Ukraine who are beneficiaries of temporary protection, the validity of the T.I.E.s is automatically extended until 04/03/2027, so it will not be necessary to obtain a new Foreigner Identity Card.

The appointment is personal, so a single appointment will not be accepted for different members of the family unit. Proof of payment of the Model 790 Code 012 fee of the Ministry of the Interior. The copy must be correctly printed in its entirety; printouts in which the lower part of the document — bearing the note "Copy for the Administration" — does not appear will not be accepted.
The form is downloaded from the Police website. THE FEE FORM CANNOT BE PHOTOCOPIED, as each form is identified by a unique identifier.
If Model 790, code 012 has been paid improperly, the refund of that fee must be processed in person at the Documentation Unit where the Foreigner Identity Card was issued, or failing that, where you obtained the appointment for that purpose.
Under no circumstances should the refund be requested through a registry.

• Formulario de solicitud modelo EX-17 (pdf), debidamente cumplimentado y firmado por el interesado o su representante legal. / Форма заявки модель EX-17 (pdf), належним чином заповнена та підписана зацікавленою стороною або її законним представником.
• Documento acreditativo de identidad. El que presentó en el momento de solicitar protección temporal. / Документ, що посвідчує особу. Той, який ви надали під час подання запиту про тимчасовий захист.
• Resguardo de solicitud de protección temporal. / Квитанція запиту про тимчасовий захист. (Para los mayores de edad, no será necesario en el caso de la TIE prorrogada).
• Debe aportar una fotografía reciente en color del rostro del solicitante, tamaño 32x26 milímetros, con fondo uniforme blanco y liso, tomada de frente con la cabeza totalmente descubierta y sin gafas de cristales oscuros o cualquier otra prenda que pueda impedir o dificultar la identificación de la persona. (La fotografía deberá mostrar claramente el óvalo de la cara, que incluye cejas, ojos, nariz, boca y mentón, y deberá ser de alta resolución y en papel fotográfico de buena calidad). / Повинен перед´явити нещодавню кольорову фотографію обличчя заявника, розміром 32х26 міліметрів, із простим білим однорідним фоном, зроблена спереду з повністю непокритою головою і без затемних окулярів або будь-який інший одяг, який може перешкодити або затруднити ідентифікацію особи. En el caso de que haya sido abonado el Modelo 790, cod. 012 de manera indebida, tendrá que ser tramitada la devolución de dicha tasa presencialmente en la Unidad de Documentación donde fue expedida la Tarjeta de Identidad de Extranjero, o en su defecto, donde obtuvo la cita con tal propósito. En ningún caso debe ser solicitada la devolución a través de registro./ У тому випадку, якщо Mодель 790, кoд 012 було oплачено неправильно, відшкодування зазначеного збору буде здійснено особисто у відділі документації, де було видано посвідчення особи іноземця, або якщо це не вдається, тo там, де він отримав призначення для цієї мети.
