Note: Submission of the medical certificate proving that the foreign national does not suffer from any disease that may have serious public health implications under the International Health Regulations of 2005 is not required in cases where, under the RLOEx, the residence authorisation is applied for while the person is in Spain.
In the case of a second marriage, proof of dissolution of the previous marriage(s). Proof may be judicial or notarial, depending on the legislation of each country.

Nota: con carácter general se deberán aportar copias de los documentos y exhibir los originales en el momento de presentar la solicitud.
• Impreso de solicitud en modelo oficial (EX – 24), debidamente cumplimentado y firmado por la persona extranjera.
• Relativos al familiar de nacionalidad española:
1.º Copia completa del pasaporte, o del documento nacional de identidad, en vigor.
2.º En los casos de solicitud a favor de cónyuge o pareja, declaración responsable de que no reside con él en España otro cónyuge o pareja.
• Relativos al familiar extranjero:
1.º Copia completa del pasaporte, del título de viaje, en vigor.
2.º Documentación acreditativa de la existencia del vínculo familiar con el familiar de nacionalidad española.
3.º En los supuestos en los que así se exija, documentación acreditativa de que el familiar se encuentre a cargo del familiar de nacionalidad española.
4.º En los casos de otros miembros de su familia (caso i. del listado de situaciones descritas anteriormente) se deberá acreditar la dependencia, la convivencia, el grado de parentesco y, en su caso, la existencia de motivos graves de salud o discapacidad.
5.º En el supuesto de pareja estable, documentación que justifique la existencia de una relación estable con la pareja de nacionalidad española, el tiempo de convivencia y, en su caso, certificado de nacimiento de los descendientes comunes.
• Certificado de antecedentes penales expedido por las Autoridades del país o países en los que haya residido durante los cinco últimos años anteriores a la entrada en España.
• Certificado médico que acredite que no se padece ninguna de las enfermedades con repercusiones en la salud pública del Reglamento Sanitario Internacional de 2005.
Nota: No es exigible la aportación del certificado médico acreditativo de que la persona extranjera no padece ninguna de las enfermedades que pueden tener repercusiones de salud pública graves de conformidad con lo dispuesto en el Reglamento Sanitario Internacional de 2005 en los casos en que, de acuerdo con el RLOEx, solicite su autorización de residencia hallándose en España.
En el supuesto de estar casado en segundas nupcias, acreditación de la disolución del(los) matrimonio(s) anterior(es). La forma de acreditación podría ser judicial o notarial, en función de la legislación de cada país.
