FAMILY MEMBERS OF EU RESIDENTS To submit the application you must attend on the day of the appointment with the application form for the Residence Card of a Family Member of a Union citizen, EX-19 (pdf), a free official form duly completed, original and copy.
NOTE: For more detailed information you should contact the Immigration Office or Unit of the relevant province. You can obtain more information at this link: Immigration Offices Information READ CAREFULLY The application must be submitted in person by the non-EU applicant before the body competent to process it, corresponding to the province where the locality in which they are registered (empadronado) is located.
If the application is made on behalf of a minor, the legal representative must submit it, and the presence of that minor in Spain must be proven. In the case of renewal, the application must be submitted during the month prior to the expiry of the residence card; it may also be submitted within the three months following that expiry date, without prejudice to any applicable administrative penalty.
Foreign certificates of family relationship must be duly translated and/or legalised.

FAMILIARES DE RESIDENTES COMUNITARIOS Para presentar la solicitud deberá acudir el día de la cita con el impreso de solicitud de Tarjeta de residencia de familiar de ciudadano de la Unión, EX-19 (pdf), modelo oficial gratuito debidamente cumplimentado, original y copia. NOTA: Para una información más detallada debe dirigirse a la Oficina o Unidad de Extranjeros de la provincia correspondiente. Puede obtener más información accediendo a éste enlace: Información Oficinas Extranjería LEE ATENTAMENTE La solicitud debe presentarse personalmente por el solicitante no comunitario ante el órgano competente para su tramitación, correspondiente a la provincia donde se encuentre la localidad en la que esté empadronado. Si la solicitud se formula a favor de un menor de edad deberá presentar la solicitud su representante legal, debiendo acreditarse la presencia en España de dicho menor. En caso de renovación a solicitud deberá presentarse durante el mes anterior a la caducidad de la tarjeta de residencia, pudiendo también presentarse dentro de los tres meses posteriores a dicha fecha de caducidad sin perjuicio de la sanción administrativa que corresponda. Los certificados extranjeros de vínculo de parentesco deberán ser debidamente traducidos y/o legalizados.
