Note: By completing the following form, you are requesting a prior appointment to submit a formal application for temporary protection for displaced persons.
If temporary protection is requested for several displaced persons belonging to the same family unit, an appointment must be requested for each family member.
Warning: by submitting this application the applicant certifies that the data provided are true and that they are the person interested in carrying out this procedure; it may under no circumstances be submitted by third parties.
Likewise, applications from persons who have already been assigned an appointment in a province other than the one selected will not be taken into account.

Nota: A través de la cumplimentación del siguiente formulario, usted está solicitando cita previa para presentar petición formal de protección temporal a desplazados.
Para el supuesto de solicitar protección temporal para varias personas desplazadas pertenecientes a un mismo núcleo familiar, se deberá solicitar una cita por cada miembro de la familia.
Advertencia: mediante el envío de esta solicitud el solicitante certifica que los datos aportados son verídicos y que es la persona interesada en la realización de este trámite, no pudiendo, en ningún caso, ser presentada por terceras personas.
A su vez, no serán tenidas en cuenta las solicitudes de aquellas personas a las que ya les haya sido asignada cita en otra provincia distinta a la seleccionada.
Para la realización del trámite de protección temporal, una vez citado, es necesario la presentación de la siguiente documentación:
• Documento de viaje y/o identidad.
• Cualquier otra documentación que pueda acreditar su identidad y nacionalidad.
• Fotografía tamaño carné con el fondo blanco.
• En caso de menores, documentación que acredite el vínculo familiar.
