You may obtain this form on the website https://www.inclusion.gob.es/documents/410169/2156469/13-Formulario_autoriz_de_regreso.pdf\n• Fee 790, code 052. The form is available at the Electronic Office of Public Administrations. https://sede.administracionespublicas.gob.es/pagina/index/directorio/tasa052\nIf you hold a residence authorisation or a stay for studies authorisation and have applied for its renewal within the legal deadline, you must also submit:\n• Complete passport or travel document or, where applicable, a valid registration certificate (cédula de inscripción).\n• Residence authorisation or stay for studies authorisation.\n• Application for renewal of residence or stay for studies.\nIf you hold a valid Foreigner Identity Card (T.I.E.) in cases of theft, loss, destruction or damage of the T.I.E.:\n• In case of theft, original and copy of the report filed with the National Police Corps or the Civil Guard.\n• Application for a duplicate card.\n• Complete passport or travel document or, where applicable, a valid registration certificate (cédula de inscripción).\nIf you hold an initial residence authorisation or an initial stay for studies authorisation, while the Foreigner Identity Card is being issued.
